Setting up your template

There are two main steps:

1. Adding your own questions to the form
2. Adding the related recommendations to the database

Using the Form builder adjust, replace and add your questions below.

2. Adding the related recommendations to the database

Preparation

The recommendations will relate to specific questions, so if you have not already, now is a good time to number your questions to more easily keep track. Depending on how many questions you have you may want to remove the text because when we are later referring to them in the formula this will make it easier to manage.
